What Is The Vision For Life Plan?

The Vision for Life Plan is an optional laser vision correction enhancement program available through West Texas Eye Associates and American Vision Partners.

If your prescription changes after LASIK or PRK, an American Vision Partners surgeon will evaluate your eyes and determine whether an enhancement is appropriate. Eligible members may receive a laser enhancement related to their original procedure without an additional surgical fee.

An enhancement is not automatic. Your surgeon must first confirm that the procedure is medically appropriate and that you continue to meet the plan’s eligibility requirements.

Vision for Life Plan Eligibility

To enroll in and remain eligible for the Vision for Life Plan, LASIK & PRK patients must:

Enroll in the plan before their surgery at West Texas Eye or with another American Vision Partners provider

Meet the required vision, prescription and eye health criteria before surgery

Have vision that could be corrected to 20/30 or better before laser vision correction

Complete all required postoperative care during the first 12 months after surgery

Remain current on payments associated with the original procedure

Complete an eye examination every year with a qualified eye care provider

Maintain documentation of annual eye examinations

Return to an American Vision Partners provider for an evaluation if their vision changes

Have any eligible enhancement performed by an American Vision Partners surgeon

 

Remain in good eye health (without eye disease)

What the Vision for Life Plan Does Not Cover

The Vision for Life Plan applies only to eligible prescription changes related to the original LASIK or PRK procedure.

It does not cover vision changes caused by:

  • Cataracts
  • Glaucoma
  • Macular degeneration
  • Diabetic eye disease
  • Retinal conditions
  • Eye injuries or trauma
  • Presbyopia or age-related difficulty focusing up close
  • Changes following cataract surgery
  • Other eye diseases, conditions, or surgeries

 

The plan also does not cover prescription medications, eyeglasses/contacts, medical devices, travel expenses, missed work or personal expenses, additional medical care, and procedures performed outside of the American Vision Partners network.

Enrollment and Cost

Eligible patients may enroll in the Vision for Life Plan before undergoing LASIK or PRK at West Texas Eye Associates. The plan has a one-time enrollment fee of $275 per eye, or $550 for both eyes.

Before your procedure, our team will explain the plan’s terms, eligibility requirements, and exclusions. You will have an opportunity to review the agreement, ask questions, and decide whether enrollment is right for you.

Yes. An annual eye examination with a qualified eye care provider is required to maintain eligibility. You should keep documentation of each examination. Annual exams also help monitor your vision and identify eye health changes that may not cause noticeable symptoms.
