Before Your Appointment

Complete Any Forms in Advance

If you received new-patient forms or questionnaires, completing them ahead of time can shorten your check-in process. If you’re unable to complete forms in advance, arrive a few minutes early so our team can assist you.

Bring These Items With You

  • A valid photo ID

  • Your vision and/or medical insurance card

  • A list of current medications (including eye drops)

  • Your current glasses or contact lenses

  • Any referral paperwork, if required by your insurance

If you’re visiting us for a specialty consultation, you may also be asked to bring prior eye records or imaging.

What to Expect During Your Visit

Your appointment may include one or more of the following, depending on the reason for your visit:

  • Vision testing and eye measurements

  • A comprehensive eye health evaluation

  • Pupil dilation, which may cause temporary light sensitivity or blurry vision

  • Time with your eye care provider to discuss findings, next steps, or treatment options

Some appointments can take longer than others, especially comprehensive or diagnostic visits. Our team will walk you through each step along the way.

Dilation & Driving

Many eye exams require dilation to fully evaluate eye health. Dilation effects can last several hours and may make driving uncomfortable for some patients. You may want to bring sunglasses or arrange transportation if you are sensitive to light or blur.
